How can I change my Primary Care Physician?
To choose a new Primary Care Physician:
  1. Locate a new Primary Care Physician either by accessing the provider directory on the network's internet web site, or by looking through a printed provider directory. You can also contact your current participating provider group for eligible physician information.

  2. Contact your carrier's Member Services department by calling the toll-free "800" number on your medical ID card. Some health care carriers allow you to change your Psptimes by sending an e-mail request to the Member Services department. Check your health care carrier's web site for more information.

    Please note that each medical plan may have different restrictions as to when your change of Primary Care Physicians will become effective.
